Fukushima: technology failure or human responsibility crisis?

Description

Despite technical progress and periodic modernization of the plant systems, accidents continue to occur at nuclear installations. Have we learned a right lesson from the past accidents? A nuclear power plant is a human-machine system, whose technological part is subject to constant upgrading so as to compensate for the adverse effects of long-term operation. But does human reliability improve adequately? Safety culture is not only a matter of the employees; first of all it is a matter to be pursued by corporate management. (orig.)
