Analysis of unusual occurrences in the field of radiography and anti-emergency measures derived therefrom

Description

In the time span from 1970 to 1979 the probability of occurrence of unusual events in gamma radiography was three times higher than in X-ray radiography. An analysis of the unusual events in the period from January 1983 through August 1985 showed that they were caused to 83% by human and 17% by technical failure. The unusual events have been classified into 5 types and measures aimed at avoiding them are discussed
