Respiratory repercussions of gastroesophageal reflux and cine-esophagogastroscintigraphy

Asthma and recurrent bronchopneumopathies in children are often associated with gastroesophageal reflux. Cine-esogastroscintigraphy is proposed for demonstrating reflux and establishing its direct or indirect responsibility in respiratory disease. 133 patients aged 5 months to 16 years were studied. Reflux was found in 65.5% of cases, either severe (44.4% of cases) or minor (21.1%). Episodes of reflux produced respiratory manifestations in 7% of patients. Inhalation of refluxed material was proved in 3% by demonstration of pulmonary contamination. Sensitivity of cine-esophagogastroscintigraphy is comparable to that of esophageal pH measurement for detection of reflux. It does not require positioning of a probe in the esophageal lumen. It provides quantitative parameters on esophageal transit, reflux and gastric voiding, and demonstrates pulmonary contamination. It is easy to perform and can be readily included in an outpatient clinic workup. Its sensitivity and reliability make it a useful tool for evaluation of therapeutic efficiency. L'asthme et les bronchopneumopathies recidivantes de l'enfant sont souvent associes a des reflux gastro-oesophagiens. La cine-oesophagogastroscintigraphie est proposee pour la mise en evidence du reflux et pour prouver sa pathogenicite respiratoire directe ou indirecte, chez 133 enfants ages de 5 mois a 16 ans. Un reflux est decouvert chez 65,5% des enfants. Il est severe dans 44,4% des cas, et mineur dans 21,1%. Les acces des reflux declenchent une symptomatologie respiratoire dans 7% des cas. L'inhalation du reflux est prouvee chez 3% par la presence d'une contamination pulmonaire. La cine-oesophagogastroscintigraphie est d'une sensibilite voisine de celle de la pHmetrie pour la mise en evidence de reflux. Elle ne necessite la mise en place d'aucune sonde dans la lumiere oesophagienne. Elle irradie nettement moins que l'etude radiologique du transit. Elle quantifie les parametres du transit oesophagien, du reflux, de la vidange gastrique, et met en evidence la contamination pulmonaire. Sa maniabilite permet facilement son integration dans un bilan d'hopital de jour. Sa sensibilite et sa fiabilite en font un moyen de mesure de l'efficacite therapeutique.
