The diagnosis of congenital pleuropericardial defects

Description

Pleuro-pericardial defects have been found incidentally during post mortems or chest surgery but, because of their usual lack of smyptoms, are rarely found otherwise. If suspicion of such a malformation arises, it should be clarified because of possible herniation of the atrium or ventricle. The necessary diagnostic steps are discussed in relation to two patients and the value of chest x-rays is stressed. A pleuro-pericardial defect should be diagnosed because of the typical radiological changes of the cardiac contour. Only in exceptional cases should it be necessary to induce a diagnostic pneumothorax or to perform an exploratory thoracotomy. (orig.)

Abstract (German)

Pleuroperikardiale Defekte sind seit langem als Zufallsbefunde bei Obduktionen und in der Thoraxchirurgie bekannt, werden aber intra vitam wegen ihres zumeist symptomlosen Vorkommens selten gefunden. Wenn der Verdacht auf eine solche Missbildung besteht, ist eine Abklaerung wegen einer moeglichen Aurikel- oder Ventrikeleinklemmung unerlaesslich. Anhand zweier eigener Faelle wird das diagnostische Vorgehen diskutiert und dabei die grosse Bedeutung des Thoraxroentgens akzentuiert. Eine pleuroperikardiale Anomalie sollte nach heutiger Ansicht dank der typischen radiologischen Veraenderungen der Herzkonturen zu verifizieren sein, und nur in Ausnahmefaellen ist die Anlage eines diagnostischen Pneumothorax oder eine explorative Thorakotomie vertretbar.
