Vncreased serum concentration of free L-triiodothyronine in patients treated with L-thyroxine

This study demonstrates that in spite of measured normal concentrations of carrier proteins one cannot deduce in all cases a normal fT3 from a normal level of TT3 when l-thyroxine given for diagnostic or therapeutic purposes is present in excess. The displacement of l-triiodothyronine from its binding sites is shown in 35 patients with non-toxic goitre who received an oral dose of 200 μg l-thyroxine/die for two weeks. Apart from a significant increase of TT4 (from 7.85 to 14.21 μg/dl corresponds to + 81%) and of fT4 (from 1.58 to 3.7 ng/dl corresponds to + 134%) there is only a slight increase in TT3 from 148 to 158 ng/dl (corresponds to + 10%) after 14 days of treatment. By contrast fT3 rises clearly from 4.97 to 8.07 pg/ml corresponds to + 63% (normal range: 2.8-5.6 pg/ml). Compared with the increase of TT3 (+ 10%) the free T3 rises by a factor of 6.3 (63%/10%). On account of higher affinity of l-thyroxine to binding proteins the free T4 is influenced to a lesser degree. Compared with the increase of TT4 (+ 81%) free T4 rises by a factor of 1.6 (134%/81%). It is supposed that the serum concentration of free T3 can be increased despite a normal concentration of TT3 when l-thyroxine is present in excess. Therefore, for laboratory work fT3 should be assigned a higher validity than TT3 when patients are treated with comparatively high doses of l-thyroxine. (orig.)
Abstract (German)
Diese Studie beabsichtigt aufzuzeigen, dass trotz gemessener normaler Serumkonzentration der Traegerproteine nicht in allen Faellen von einem normalen TT3-Serumspiegel auf ein im Normbereich gelegenes fT3 geschlossen werden kann, wenn L-Thyroxin (appliziert aus diagnostischer oder therapeutischer Absicht) im Ueberschuss vorhanden ist. Die Verdraengung des L-Trijodthyronin aus seinen Bindungsplaetzen wird gezeigt an 35 Patienten (Durchschnittsalter: 36,4 Jahre), die im Rahmen eines Suppressionstests ueber 14 Tage 200 μg L-Thyroxin/die erhielten. Nach 14taegiger Behandlung laesst sich gegenueber einem signifikanten Anstieg des TT4 (von 7.85 auf 14.21 μg/dl corresponds to + 81%) bzw. fT4 (von 1.58 auf 3.7 ng/dl corresponds to + 134%) nur ein leichter Anstieg des TT3 von 148 auf 158 ng/dl (corresponds to + 10%) beobachten. Im Gegensatz zum TT3 steigt das freie T3 kraeftig an von 4.97 auf 8.07 pg/ml corresponds to + 63%. (Normalbereich: 2.8-5.6 pg/ml). Eine Proteinbindungsanalyse macht deutlich, dass sich nach 14taegiger Behandlung der Prozentsatz des an TBG gebundenen L-Trijodthyronins und L-Thyroxins von 79.6 auf 70.53% bzw. von 68.1 auf 56.7% reduziert zugunsten einer Bindung an TBPA und Albumin. Verglichen mit dem Anstieg des TT3 (+ 10%) steigt das freie T3 um den Faktor 6.3 (63%/10%). Aufgrund der hoeheren Affinitaet des L-Thyroxins zu den Bindungsproteinen ist fT4 in geringerem Ausmass beeinflusst: Verglichen mit dem Anstieg des TT4 (+ 81%) steigt das freie T4 um den Faktor 1.6 (134%/81%). Die Ergebnisse lassen annehmen, dass die Serumkonzentration des freien T3 erhoeht sein kann trotz eines normalen TT3, wenn L-Thyroxin im Ueberschuss vorhanden ist. Damit sollte in der Labordiagnostik dem freien T3 eine hoehere Validitaet als dem Gesamt-T3 zugeordnet werden, wenn Patienten mit vergleichsweise hohen Dosen L-Thyroxin therapiert werden.
