Root cause analysis: techniques and applications

This paper shows some of the existing techniques for the analysis of undesired events, at nuclear plants or other plants or systems not related to it, allowing to identify the most deep cause of accidents or operational problems, reducing, removing and preventing its recurrence, bringing improvement in quality and productivity and also reducing the costs of processes. This study was developed taking some examples of case study and will be continued in the analysis of some abnormal events at a research as part of a master thesis research. (author)
