RIA for calcitonin detection in human serum

Description

A RIA for human calcitonin (HCT) was adapted to the microtiter system. The method had a sensitivity of 60 pmol/l which is, however, not sufficient for measurements of the HCT level in healthy persons. Oral and i.v. administration of calcium, oral administration of vitamin D, and glucose tolerance tests did not result in a calcitonin increase over the detection limit. In two patients with histologically confirmed medullary thyroid carcinoma, there are extremely increased calcitonin levels: 16,000 and 120,000 pmol/l. In two other patients with medullary thyroid carcinoma after total resection of the thyroid, HCT levels were below the detection limit. In patients with hypercalcaemia (tumor hypercalcaemia), increased HCT levels were not observed. On the other hand, a HCT value of 150 pmol/l was found in a patient with vitamin D intoxication. As is normal in tumour cases of this kind, the two patients with medullar thyroid carcinoma examined during the study suffered from severe diarrhoea. In both cases, the family anamnesis showed that the mothers of the two patients had also suffered from thyroid cancer. (orig.)

Ein Radioimmunoassay fuer Humancalcitonin (HCT) wurde auf das Mikrotiter-System adaptiert. Es ergab sich eine Empfindlichkeit von 60 pmol/l, die jedoch noch nicht ausreichend ist, um die HCT-Spiegel bei Gesunden zu messen. Durch orale Calcium-Zufuhr und i.v. Calcium-Zufuhr, ferner orale Vitamin-D-Zufuhr und die Durchfuehrung von Glukose-Toleranz-Testen wurden keine Anstiege des Calcitonins ueber die Nachweisgrenze hinweg erzielt. Bei zwei Patientinnen mit histologisch nachgewiesenem medullaeren Schilddruesencarcinom wurden extrem erhoehte Calcitonin-Spiegel gemessen: 16 000 und 120 000 pmol/l. Bei zwei weiteren Patienten mit medullaerem Schilddruesencarcinom nach totaler Thyreoidektomie wurden nicht messbare HCT - Spiegel festgestellt. Bei Patienten mit hypercalciaemischen Erkrankungen (Tumorhypercalciaemie) liessen sich keine Anstiege des HCTs nachweisen. Bei einer Patientin mit Vitamin D-Intoxikation wurde jedoch ein HCT-Wert von 150 pmol/l gefunden. Die zwei in dieser Arbeit untersuchten Patientinnen mit medullaerem Schilddruesencarcinom wiesen, wie haeufig fuer diesen Tumor berichtet, starke Durchfaelle auf. Bei beiden Patientinnen wurde in der Familienanamnese ein Schilddruesencarcinom der Muetter eruiert. (orig.)
