Nuclear medicine in chronic infections

Description

Background. The availability of high sensitive radiopharmaceuticals for the diagnosis of an unknown focus of infection with whole body scanning and SPECT and the availability of high specific radiopharmaceuticals for the detection of a leucocytic infiltration are the basis of the high potential of nuclear medicine in patients with chronic infections. Both procedures are always complementary to radiological procedures, which can specify and better determine a detected focus and which can induce also infection imaging if the radiologic result is ambiguous. Radiopharmaceuticals. Not all available radiopharmaceuticals for the nuclear medicine imaging of infection and inflammation, i.e. 99mTc-nanocolloids, 99mTc-labelled human immunoglobulin, 99mTc-HMPAO and 111In-oxin labelled leukocytes, 67Ga-citrate, 99mTc-labelled antigranulocyte anitbodies and their fragments and 18F-FGD-PET, are useful in all clinical situations, because there are differences in the availability, the radiation exposure, the cost and the underlying patho-physiological mechanism. Abstract (German)

Hintergrund. Die Bedeutung der nuklear-medizinischen Diagnostik bei chronischen Entzuendungen liegt einerseits in der Verfuegbarkeit hochsensitiver Radiopharmazeutika, die bei unklarem Fokus einen Entzuendungsherd lokalisieren lassen, da die Moeglichkeit der Ganzkoerperszintigraphie und der SPECT-Technik gegeben ist, und andererseits in der Verfuegbarkeit hochspezifischer Radiopharmazeutika, die z.B. den Beweis einer leukozytaeren Infiltration ermoeglichen. Beide Verfahrensansaetze sind jeweils komplementaer zu roentgenmorphologischen Verfahren, die einen gefundenen Fokus naeher spezifizieren koennen oder die eine Entzuendungsszintigraphie bei unklarem Befund nach sich ziehen. Radiopharmaka. Nicht alle fuer die Entzuendungsszintigraphie verfuegbaren Radiopharmaka, wie 99mTc-Nanokolloide, 99mTc-markiertes humanes Immunglobulin, 99mHMPAO-, 111In-Oxin-markierte Leukozyten, 67Ga-Zitrat, 99mTc-Antigranulozytenantikoerper oder deren Fragmente und die 18F-FDG-PET, koennen fuer jede klinische Fragestellung zum Einsatz kommen, da die Verfuegbarkeit, die Strahlenexposition, die Kosten und die pathophysiologischen Gegebenheiten zu beruecksichtigen sind.
