Radio-nuclide phlebography: Methods, indications and clinical value

Description
Radionuclide venography (RNV) is an easily performed, rapid and non-invasive diagnostic examination for the detection of deep-vein thrombosis. Provided the procedure is followed out in a standardized order, the results are highly reliable and accurate, especially in the iliac and femoral viens. Therefore, because of this advantage and the possibility of simultaneous lung perfusion scanning and the lack of serious complications this method fills a gap between contrast phlebography and fibrinogen test. 122 limbs of 69 patients were investigated by RNV. 65% showed pathologic results. In 48% thrombotic venous occlusions could be demonstrated. Pulmonary embolic perfusion defects wer found in 46% of patients presenting deep vein thrombosis but only in 15% of patients without pathologic findings in RNV. In correlation with contrast phlebography in 11 patients there was no discrepancy neither in recognition nor in localization of the thrombotic occlusion. The essential findings in RNV and the resulting diagnostic conclusions are demonstrated. (orig.)
Abstract (German)
Die Radionuklidphlebographie stellt ein einfach und rasch durchzufuehrendes, nicht-invasives Verfahren zum Thrombosenachweis dar. Eine standardisierte Untersuchungstechnik vorausgesetzt, besitzt sie eine hohe diagnostische Treffsicherheit vor allem im Oberschenkel- und Beckenbereich. Aufgrund dieses Vorteils, ferner wegen der fehlenden Nebenwirkungen und nicht zuletzt durch die Kombinationsmoeglichkeit mit der Lungenperfusionsszintigraphie fuellt diese Methode eine Luecke zwischen Roentgenphlebographie und Radiofibrinogentest. In unserem Untersuchungsgut von 122 Bein-Radionuklidphlebographien fanden sich in 65% pathologische Befunde. In 48% konnte ein direkter Verschlussnachweis gefuehrt werden. 46% der Patienten mit thrombotischen Beinvenenverschluessen, aber nur 15% derer mit unauffaelligem Radionuklidphlebogramm hatten Lungenembolien. Im direkten Vergleich mit der Roentgenphlebographie an 11 Patienten zeigte sich keine Diskrepanz bezueglich Nachweis und Lokalisation des venoesen Verschlusses. Die wesentlichen radionuklidphlebographischen Befundkonstellationen und die daraus ableitbaren Informationen werden demonstriert. (orig.)Additional details
