Description
As a result of CT guidance, thoracic biopsy has become a very reliable procedure in diagnosis and pre-treatment staging. The review of 306 cases revealed an overall accuracy of 84% with 9% technical failures and 6% false negatives. The authors report their own technique and emphasise the value of atraumatic biopsy material: fine needles and screw needles. CT and fine needles greatly reduce the risk of pneumothorax and negative aspiration. Good collaboration between radiologists, penumologists and cytologists is an essential prerequisite for this method of investigation. The authors discuss the indications for CT guided aspiration biopsy and propose a diagnostic plan, which must include fibroscopy even in the case of a peripheral mass, readily accessible to percutaneous biopsy
Abstract (French)
Grace au guidage TDM la biopsie thoracique devient un geste de haute fiabilite. Sur 306 cas les auteurs enregistrent 84% de fiabilite globale avec 9% d'echecs techniques et 6% de faux negatifs. La ponction-biopsie peut ainsi prendre toute sa place dans la chaine diagnostique et dans les bilans pre-therapeutiques, quand la fibroscopie est negative. Les auteurs font etat de leur technique. Ils montrent l'interet d'un materiel peu traumatique pour biopsier: aiguilles fines, millimetriques et notamment aiguilles rotatives. La TDM et les aiguilles fines diminuent le risque de pneumothorax et de ponction blanche de facon spectaculaire.
