Function of the anterior pituitary gland following surgical and radiotherapeutical management for pituitary adenoma

Description
Fifty-five adult patients with adenoma of the pituitary gland (20/55 with hyperfunctional tumors, 21/55 with endocrine inactive tumors, and 14/55 with hormonal deficits at the time of diagnosis) were studied retrospectively in terms of their endocrine outcome after surgery and radiotherapy. Twenty-two percent of the patients developed new hormone deficiency after surgery and 33% after radiotherapy. After a median follow-up time of 4.6 years, 44/53 patients had a pituitary dysfunction and 35/44 a gonadotropin deficiency. There seems to be a correlation between the daily single dose given and the development of endocrine dysfunction, as 15/35 (43%) of those patients who received 2.0 Gy per day developed hormonal deficiencies, compared to 6/20 (30%) of those who received 1.8 Gy per day. The median time from radiotherapy to onset of endocrine abnormalities was 19.7 months. Normalization of the prolactin levels occurred in 55% of the cases, and that of hGH levels in 80%. An ongoing hormone replacement was necessary in 69% of the patients. (orig.)

Bei 55 erwachsenen Patienten mit Hypophysenadenomen, davon 20 mit hormonproduzierenden Adenomen, 14 mit insuffizienter Hormonproduktion und 21 mit praetherapeutisch unauffaelligen Hormonwerten, wurden retrospektiv die therapiebedingten Aenderungen der hormonellen Hypophysenfunktionen untersucht. Zweiundzwanzig Prozent der Patienten entwickelten postoperativ und 38% nach der Strahlentherapie einen bleibenden neuen bzw. zusaetzlichen Hypopituitarismus. Nach einer medianen Beobachtungszeit von 4,6 Jahren hatten 43/55 Patienten eine hypophysaere Dysfunktion, davon 35 Faelle eine gonadotrope Insuffizienz. Die Hoehe der Einzeldosis scheint fuer das Entstehen von Dysfunktionen mit ausschlaggebend zu sein: 15/35 (43%) der Patienten, die mit 2,0 Gy taeglich bestrahlt wurden, entwickelten nach der Bestrahlung eine hormonelle Stoerung, vs. 6/20 (30%) der Patienten mit 1,8 Gy taeglich. Vom Ende der Radiation bis zum Auftreten der Dysfunktion vergingen im Mittel 19,7 Monate. Eine Normalisierung der Hyperprolaktinaemie wurde bei 55%, der Wachstumshormonueberproduktion bei 80% der Faelle erreicht. Eine andauernde medikamentoese Substitutionstherapie benoetigten 69% der Patienten. (orig.)Additional details
