Description
New clinical and experimental investigations have shown that malignant melanomas are not radioresistant. Their radiosensitivity enables definitely curative radiotherapy with moderately high doses. Success is more dependent on duration of treatment than with other tumors. The position of radiation therapy in primary curative treatment of malignant melanoma should be reconsidered on the basis of these new findings. In the treatment of occult regional metastases irradiation is at least equivalent to lymph-adenectomy unless the diagnosis is decisive. If lymph node metastases are manifest, irradiation should be combined with lymphadenectomy. Irradiation of distant metastases may be given palliatively, especially for the purpose of analgesie. Abstract (German)
Neue klinische und experimentelle Untersuchungen haben gezeigt, dass maligne Melanome nicht strahlenresistent sind. Ihre Strahlenempfindlichkeit ermoeglicht durchaus eine kurative Strahlentherapie mit maessig hohen Dosen. Der Erfolg ist staerker abhaengig von der Dauer der Behandlung als bei anderen Tumoren. Die Stellung der Strahlentherapie bei der kurativen Primaerbehandlung des malignen Melanoms sollte auf Grund dieser neuen Erkenntnisse ueberdacht werden. Bei der Behandlung der okkulten regionaeren Metastasen ist die Bestrahlung der Lymphadenektomie mindestens gleichwertig, wenn es nicht auf die Diagnostik ankommt. Bei manifesten Lymphknotenmetastasen sollte die Bestrahlung mit der Lymphadenektomie kombiniert werden. Die Bestrahlung von Fernmetastasen kann palliativ insbesondere zur Analgesie erfolgen.
