A coincident instrument failure results in overpower operation of a TRIGA reactor

Description

Removal of an annular, dry irradiation facility from the reflector region of a TRIGA Mk II reactor caused an unanticipated reduction of ion chamber sensitivity of a factor-of-two. Available reactivity and control rod worth and calibration were essentially uneffected. Loss of sensitivity of power indicating channels combined with malfunction of pulse rod position indicating lights resulted in a brief operation about 60% above licensed power level. (author)
