Proposed study to identify the obstacles to wearing a fluoride splint in patients following radiotherapy to the cervicofacial sphere or in the context of Gougerot Sjoegren's syndrome
Description
Hyposialia represents a major challenge to oral health. Saliva possesses protective attributes thanks to its mechanical and chemical properties on dental health as well as gum and mucous tissues. Its absence causes multiples lesions such as tooth decay, generating severe structural loss which require complex global dental care. Hyposialia is a clinical symptom found in patients who have been treated with orofacial sphere radiotherapy but also those affected by Gougerot-Sjoegren's disease. Beyond tooth decay treatment in these patients, it is important to prevent their occurrence as well as their recurrence during all the patients' lives. It is the reason why prevention methods, especially thanks to chemical agents such as fluoride, must be implemented and generalised in patient care. Thus, the fluoridation dental tray is systematically prescribed in patients who have been treated by oro-facial sphere radiotherapy, during and after treatment. This practice is less widespread in patients affected by Gougerot-Sjoegren's disease even though it has been recommended.The goal of this work is to lead a transversal investigation amongst patients where an indication for this dental tray exists, to evaluate why these patients have not benefited from this treatment or have not used it. This will allow to improve this prevention method's implementation. Investigation will be led to a patient's cohort recruited within Dr. Florimond and Fouilloux's oncology consultation ward, Dr. Decup and Gosset's dry syndrome and Gougerot-Sjoegren's disease consultation ward at Hopital Charles Foix, the Rheumatology department at Hopital Bicetre in Paris, the dry syndrome and Gougerot-Sjoegren's disease French association, and the oro-facial oncology department of Hopital Gustave Roussy in Paris. This work will describe the development of the form presented to patients and implementation of reglementary protocols to lead this investigation. (author)

L'hyposialie represente un enjeu majeur pour la sante bucco-dentaire. En effet, la salive presente des effets protecteurs grace a ses actions mecaniques et chimiques sur la sante dentaire ainsi qu'au niveau des muqueuses et de la gencive. Son absence engendre chez les patients de multiples lesions, entrainant des pertes de structures severes impliquant une prise en charge globale complexe.L'hyposialie est un symptome clinique retrouve chez les patients ayant ete traites par radiotherapie de la sphere cervico-faciale mais aussi atteints du Syndrome de Gougerot Sjoegren. Chez ces patients, il est important de prevenir leur survenue ainsi que leur recidive tout au long de la vie des patients. Ainsi, la gouttiere de fluoration est systematiquement prescrite chez les patients ayant ete traites par une radiotherapie de la sphere cervico-faciale pendant et apres leur traitement. D'apres notre experience, cette prevention semble moins systematisee chez les patients atteints du syndrome de Gougerot Sjoegren bien qu'elle soit recommandee. L'objectif de ce travail est, apres avoir expose l'etat des connaissances sur la fluoration dans le cadre de l'hyposialie associee a une irradiation des glandes salivaires ou d'un syndrome de Gougerot Sjoegren, de proposer une enquete transversale pour identifier les freins au port de ces gouttieres. L'enquete sera diffusee aupres des patients recus au sein de la consultation de cancerologie des Dr Florimond et Fouilloux, de la consultation Gougerot Sjoegren et syndromes secs des Dr Decup et Gosset au sein de l'Hopital Charles Foix, du Service de Rhumatologie de L'hopital Bicetre a Paris, de services medicaux partenaires de ses consultation tels que le service de cancerologie cervico-faciale de l'hopital Gustave Roussy a Paris et de l'association Francaise du Gougerot Sjoegren et des syndromes secs. Ce travail decrira le developpement du questionnaire et la mise en oeuvre des procedures reglementaires pour pouvoir mener l'enquete. (auteur)
