Highlights: • HU values in spine CT enable preliminary detection of abnormal bone mass density. • HU values on sagittal and axial CT are comparable. • Probability of abnormal BMD is low if HU > 160; high if HU ≤ 110; intermediate between. • Routine reporting of lumbar vertebrae HU values in CT all reports is warranted. We aimed define thresholds for HU values observed on opportunistic CT scans that suggest abnormal bone mineral density (BMD) in a heterogeneous Middle Eastern population.
