Bone scintigraphy in the diagnosis of bone invasions of face and skull epitheliomas and ORL neoplasms
Description
Bone scintigraphy has been considerably improved by the use of technetium 99m-labelled organic phosphates: the physical characteristics are almost ideal for detection and the extemporaneous labelling of the product is convenient. This study on 75 patients has proved the value of bone scintigraphy in evaluating the bone invasion of tumoral lesions affecting the face and skull. An 88% agreement was found for scintigraphy, radiography giving 81%. The technique allows a fast assessment of whole-body bone invasion from one examination, with no extra injection of radioactive product, and it is therefore possible to orient the radiological enquiry without having to X-ray the whole skeleton, which reduces the amount of irradiation delivered to the organism. From these observations it may be concluded that scintigraphy is a simple, harmless method, with many advantages, but its limits must be known and it must always be interpreted as a function of other clinical and radiological data. It should be systematic in research on the local spreading of face and skull epitheliomas, in neoplasms of the ORL sphere and in the search for bone metastases

L'utilisation des phosphates organiques marques au 99m Technetium a ete un progres considerable pour la scintigraphie osseuse: les caracteristiques physiques sont voisines de l'ideal pour la detection, et le marquage extemporane du produit est de realisation commode. A la suite de cette etude portant sur 75 malades, nous pouvons affirmer la place et l'interet de la scintigraphie osseuse dans le bilan d'extension a l'os de lesions tumorales situees au niveau de la face et du crane. Nous avons trouve une concordance de 88% pour la scintigraphie, alors qu'elle est de 81% pour la radiographie. Elle permet de realiser un bilan rapide d'extension osseuse corporelle totale autour du meme examen sans qu'il soit necessaire d'injecter une quantite supplementaire de produit radioactif et permet donc d'orienter l'enquete radiologique sans qu'il soit utile d'effectuer une radiographie totale du squelette, reduisant ainsi l'irradiation de l'organisme. De ces constatations, nous pouvons deduire que la scintigraphie est un examen simple, peu irradiant, presentant de nombreux avantages, mais il faut en connaitre les limites et toujours l'interpreter en fonction des autres donnees cliniques et radiologiques. Elle doit etre systematique dans la recherche de l'extension locale des EOA cutanes de la face et du crane, dans les neoplasmes de la sphere ORL et dans la recherche de metastases osseusesFiles
