Puncture of a sealed radioactive source

Description

Most accidents involving a borehole-logging sealed radioactive source occur when a source becomes stuck in a hole deep underground. However, this paper describes an accident in which a borehole-logging source was damaged in a workshop. The following description of the accident and the management during the clean up may provide a useful insight for others who deal with accidents.
