Faulty installation of expansion bolts in Swedish nuclear power plants

Description

In five nuclear power units in Sweden defective expansion bolts have been detected in large numbers. When installing the bolts the worker have encountered reinforcement steel in the concrete and have sawn off the bolts to ease installation. Piping supports of a detective design have also had to be replaced. The question of responsibility for quality control is discussed. (JIW)
