31P magnetic resonance spectroscopy in the follow-up of therapy of patients with astrocytomas

MR image-guided 31P MR spectra of the brain were obtained from 12 astrocytoma patients who underwent irradiation and surgical therapy and six months. Seven of the patients received a follow-up examination, typically after 6-10 weeks. The six normals were examined a total of three times each, with two weeks between each examination. The patient group was split into four responders and three non-responders on the basis of clinical information. It was found that responders showed a decrease of total 31P signal of greater than 30% following radiotherapy whereas non-responders showed smaller decreases (less than 20%) or increases in total 31P signal. Total 31P signal was a very reproducible and stable parameter in the normals. The results suggest that total 31P signal might be a potentially useful parameter in assessing the effect of radiotherapy upon brain tumors and the unifiltrated hemisphere. (orig.)

Abstract (German)

MR-tomographische 31P-Spektren des Gehirns wurden von 12 Patienten mit Astrozytomen nach Operation und Bestrahlung und von 6 normalen Kontrollpersonen aufgenommen. Sieben Patienten wurden 6-10 Wochen spaeter nachkontrolliert. Die 6 Probanden wurden jeweils dreimal im Abstand von 2 Wochen spektroskopiert. Aufgrund klinischer Daten erfolgte eine Aufteilung der Patienten in 4 'Responder' und 3 'Non-Responder'. Bei Ansprechen der Therapie zeigte sich ein mehr als 30%iger Abfall des 31P-Signals nach Bestrahlung. Fehlendes Ansprechen ging mit geringeren Abfaellen (<20%) oder Anstiegen des 31P einher. Das 31P-Gesamtsignal erwies sich bei den Probanden als sehr konstant und reproduzierbar. Die Ergebnisse legen nahe, dass das 31P-Gesamtsignal moeglicherweise ein gut verwertbarer Parameter ist, um den Effekt einer Strahlentherapie bei Hirntumoren und im mitbestrahlten Gehirn abzuschaetzen.
