Value of skeletal scan in pretherapeutic staging of head and neck tumors

Description
Bone scans were performed in 46 patients with advanced squamous carcinomas of the head and neck. The tumour size was classified as T3 in 33 patients and T4 in 10 patients. Besides 3 patients suffered from advanced tumors of the paranasal sinuses. Bone metastases were seen in 3 cases. 10 patients with T3 and 8 with T4 tumors were found with pathological local findings. Conventional radiographs showed osteolytic lesions in only 4 cases. Since the majority of our patients with tumors of the epipharynx, paranasal sinuses, and floor of the mouth showed pathological radionuclide accumulations, bone scanning is considered a useful diagnostic procedure for staging and treatment planning of advanced head and neck tumors. Abstract (German)
Bei 46 Patienten mit fortgeschrittenen Kopf-Hals-Tumoren wurden Szintigramme des gesamten Skelettsystems und des Schaedels angefertigt. 33 Patienten wiesen ein T3-Stadium, 10 ein T4-Stadium und 3 fortgeschrittene Nasennenbenhoehlentumoren auf. Bei nur 3 Patienten liessen sich Skelettmetastasen finden. 10 T3 und 8 T4-Tumoren wiesen einen pathologisch lokalen Knochenbefund auf. In 4 Faellen zeigten herkoemmliche Roentgenuntersuchungen knoecherne Strukturveraenderungen. Da die Mehrzahl der Mundboden-, Epipharinx- und Nasennebenhoehlentumoren pathologische Nuklidakkumulationen aufwiesen, ist die Skelettszintigraphie als eine sinnvolle ergaenzende Massnahme zur Stadieneinteilung und Therapieplanung anzusehen.
