Development of a kit for RBC labelling with 99mTc and its clinic evaluation

Description
A kit for labelling red blood cells (RBC) with 99mTc based on a Tin Pyrophosphate mixture freeze-dried and a saline solution saturated with nitrogen has been prepared for spleen and placenta scanning, circulatory studies and blood volume determinations. The stannous tin is intended to reduce the 99mTc pertechnetate, obtained either from generators or from solvent extraction, to the appropriate valence state suitable for labelling. The technique is as follows: Add to the heparinized blood sample the Tin-Pyrophosphate mixture dissolved in the saline solution. After five minutes incubation time at room temperature the plasma is discarded by centrifugation. Following the addition of the tracer activity to the separated RBC, the supernatant is discarded after a second centrifugation. Depending on the test to be performed, the RBC are either resuspended in saline solution for blood pool or circulation studies or denaturalized at 49.5 deg C for spleen scanning. The labelling yield (95-88%) and its stability has been checked by in vitro measurements, up to four hours after labelling. The biological distribution and spleen uptake have been determined in rats and rabbits. The developed method proved to be adequate for labelling RBC with good labelling retention. An interesting application is shown in babies with congenital cardiac defects and suspected absence of the spleen, where the scan gave a good image of the existing spleen. (author)

Se estudio la marcacion de globulos rojos con 99mTc para estudios circulatorios, determinacion de volemia y centellografia esplenica y placentaria. Se puso a punto un juego de reactivos que consiste en un liofilizado de pirofosfato-estano. El estano reduce 99mTc, proveniente de generadores o de soluciones de alta concentracion de actividad, a un grado de valencia tal que permite su fijacion en los globulos rojos. Se provee tambien solucion fisiologica nitrogenada para la disolucion del liofilizado. Todas las preparaciones se realizan bajo atmosfera de nitrogeno y en condiciones de esterilidad. La tecnica de marcacion es la siguiente: A la sangre extraida y heparinizada se le agrega el pirofosfato-Sn disuelto en la cantidad apropiada de solucion fisiologica nitrogenada. Luego se incuba 5 minutos a temperatura ambiente, se centrifuga y descarta el plasma. Al paquete de globulos rojos se le incorpora un trazador, se centrifuga y descarta el sobrenadante. Segun se lo utilice luego para pool sanguineo o localizacion esplenica, se lo resuspende en solucion fisiologica, o se lo incuba para desnaturalizarlos a 49,5 grados C y se lo reinyecta. Se realizaron controles in-vitro de rendimiento (95-88%) y de la estabilidad de la marcacion hasta 4 horas despues de la marcacion, y se determino la distribucion biologica y la localizacion esplenica en animales de experimentacion. El metodo demostro ser adecuado para la marcacion y dio una buena retencion. Se describe una interesante aplicacion en ninos con cardiopatias congenitas y con supuesto sindrome de asplenia, para localizar la presencia de bazo (micro o poliesplenia). (autor)Files
