Cranial computed tomography in psychiatry

Description
Computed tomography has gained importance as a diagnostic tool in psychiatry to exclude structural brain pathology, but has passed on its role in research to magnetic resonance tomography. It helps to distinguish between senile dementia of Alzheimer type and multi-infarct dementia. The enlargement of the ventricular system and cortical sulci is well established in schizophrenic and affective psychosis. Some alcohol addicts show a considerable degree of cerebral atrophy, only exceeded by demented patients, but this condition is potentially reversible. To screen psychiatric patients by CT is recommendable, as 2-10% of hospitalized psychiatric patients have structural brain disease. (orig.)
Abstract (German)
Die kraniale Computertomographie hat in der psychiatrischen Diagnostik grosse Bedeutung fuer den Ausschluss hirnorganischer Veraenderungen, verliert demgegenueber in der Forschung gegenueber der Kernspintomographie zunehmend an Terrain. Fuer die Demenzen ist die zur Differenzierung zwischen der Senilen Demenz vom Alzheimertyp und der Multiinfarktdemenz von Bedeutung. Eine Erweiterung des Ventrikelsystems und der kortikalen Sulci konnte fuer schizophrene und affective Psychosen bestaetigt werden. Eine Subgruppe Alkoholiker weist ein Ausmass an zerebraler Atrophie auf, das nur von Dementen ueberschritten wird, aber potentiell reversibel ist. Eine Inzidenz von 2 bis 10% hirnorganischer Veraenderungen bei stationaeren psychiatrischen Patienten laesst die Durchfuehrung eines Computertomogramms als Screening-Methode als durchaus empfehlenswert erscheinen.
