A survey performed in 2013 by the SFPM revealed that 52 radiotherapy centres (98 pc of respondents) declared record errors in the R and V (Record and Verify) system. Even though no consequence has been noticed, the risk is to exceed the prescribed dose. As the R and V is a system supposed to secure treatment parameters, this publication addresses record defects of treatment parameters, excluding problems of transfer of the treatment planning system (TPS) towards the R and V, and manual input errors. The publication proposes an assessment of surveys performed among users and manufacturers, reports the experience of two centres, and proposes an analysis of possible consequences of these dysfunctions.
