Osteoradionecrosis following irradiation with cobalt-60 in the head-neck region

Description

Of 628 patients who had undergone 60Co treatment for malignomas in the head-neck region during a period of 8 years, 17 developed osteoradionecrosis. In 14 cases, the lower jaw was affected, while the upper jaw, mastorid, and larynx were affected each in one case. The decisive factor for the development of osteoradionecrosis is the focal dose which was between 6200 and 8000 R in the cases studied here. In most patients, osteoradionecrosis manifested itself within the first two years, but there were also latency periods up to 9 years. The importance of dental treatment before radiotherapy is stressed. (MG)

Abstract (German)

Von 628 Patienten, die in einem Zeitraum von 8 Jahren wegen eines Malignoms im Kopf-Hals-Bereich mit 60Co bestrahlt wurden, entwickelten 17 eine Osteoradionekrose, wobei in 14 Faellen der Unterkiefer, in je einem Oberkiefer, Mastoid und Kehlkopf betroffen war. Wesentlich fuer die Ausbildung einer Osteoradionekrose ist die veabfolgte Herddosis, die hier zwischen 6200 und 8000 R lag. Die Osteoradionekrose manifestierte sich bei den meisten Patienten innerhalb der ersten 2 Jahre, jedoch kamen auch Latenzzeiten bis zu 9 Jahren vor. Es wird auf die Bedeutung einer Zahnsanierung vor Beginn der Strahlentherapie hingewiesen. (MG)
