Highlights: • Kinematic assessment of ankle syndesmosis using 4D CT examination is feasible. • Syndesmotic measurements could be obtained during ankle motion with excellent inter-observer reliability. • Syndesmotic translation decreases during plantar flexion, but the remaining measurements remain unchanged during ankle motion. -- Abstract: To evaluate the reliability of ankle syndesmotic measurements and their changes during active motion using four-dimensional computed tomography (4DCT) examination in asymptomatic ankles.
