Whereas it is possible to qualitatively and quantitatively measure the blood flow rate, brain energy metabolism and their local correlations by using PET with various tracers, some data are accessible or not accessible for humans or for animals when using a continuous inhalation of oxygen-marked gases coupled with PET. This thesis aimed at proposing an approach to values of the regional brain blood flow rate and of the brain energy metabolism represented by the regional oxygen consumption, by using the method of continuous inhalation of gases marked with oxygen 15. It also reports the study of influencing factors. Values are discussed with respect to a theoretical model and to difficulties related to technical application. Reference normal values are proposed for individuals with a normal brain, and this allows the assessment of a focalized lesion. The flow rate/metabolism pair, and age effects on measured parameters are discussed
Abstract (French)
Ce travail a pour but de proposer une approche des valeurs de debit sanguin cerebral regional (DSCr) de consommation d'oxygene regionale (rCMRO2) par l'utilisation de la methode d'inhalation continue de gaz marques a l'oxygene 15, et l'etude des facteurs intervenant dans les resultats. Les valeurs de DSCr, de rCMRO2 et le taux d'extraction d'O2 (EO2) sont mesurees dans des regions d'interet placees au sein de zones cerebrales a dominance de substance grise ou blanche. Ces valeurs seront discutees en fonction des limites du modele theorique utilise et aussi des difficultes d'application technique. Des valeurs normales de reference seront proposees pour une serie de 19 sujets a cerveau normal, avec un intervalle de confiance a 95 %, ce qui permettra l'evaluation d'une lesion focalisee en pathologie. Le couple debit/metabolisme et les effets de l'age sur les parametres mesures seront aussi commentes. La technique d'inhalation continue d'oxygene 15 couplee a la tomographie d'emission de positons est actuellement la seule technique suffisamment validee qui permette d'etudier localement le DSCr et la rCMRO2 et cela de facon atraumatique. Ce travail permet d'etablir des donnees de reference pour l'etude des affections neuro-psychiatriques mais aussi de montrer que malgre la complexite de l'infrastructure exigee, la quantification est realisable de facon tout a fait satisfaisante par la methode 15OFiles
