Search and finding of a brachytherapy source

Description
On January 27, 2003, a source of Cs-137 1,45 GBq, which was used for brachytherapy in a hospital in Lima city was lost. The notification of the event was made after 5 days of the detection of the loss, and then the search was initiated. The source was found in a dump in Lima, 3 days after the notification. The maximum dose estimated for a person located 10 cm from the source for two hours was 22 mSv and for the staff who participated in the recovery of the source was 3,1 mSv. (author)
