Review of photodynamic therapy with 5-methyl aminolevulinate in actinic keratosis, epidermoid carcinoma and basal cell carcinoma

A bibliographic review was conduced on the use of 5-methyl aminolevulinate in dermatology, specifically in the treatment of actinic keratosis, epidermoid carcinoma and basal cell carcinoma. The basic fundamentals of photodynamic therapy are described. The preparation and method of use of photodynamic therapy with 5-methyl aminolevulinate (MAL-PDT) are detailed. The clinical studies that were realized with photodynamic therapy for the treatment of actinic keratosis, epidermoid carcinoma and basal cell carcinoma are mentioned. Different photo-inducible agents and other current therapeutic options of first-line are compared. The MAL-PDT has have the advantage of to present less side effects and the same have been more tolerable than liquid nitrogen and 5 fluorouracil. The MAL-PDT has been considered as an effective option for the treatment of Bowen's disease. Invasive epidermoid carcinoma has existed without evidence to support the routine use of this therapeutic. For superficial basal cell carcinoma, the MAL-PDT has presented a high cure rate and transient and manageable side effects in extensive and multiple lesions. The MAL-PDT has been an effective and safe treatment in patients with basal cell carcinoma, for those with less depth of 2mm. The MAL-PDT could play an important role in the field of prevention with immunosuppressed patients, particularly, those that have required transplant and its immunosuppression has been pharmacological. The use or not of the MAL-PDT, should be evaluated individually for each patient and to have suitable characteristics for each disease that was cited in this review. The photodynamic therapy with 5-methyl aminolevulinate has been a therapeutic modality of considerable economy, however, it should be evaluated in the context of number of inquiries and side effects that have offered other therapeutic modalities

Una revision bibliografica se ha realizado sobre el uso de 5-metil aminolevulinato en dermatologia, especificamente en el tratamiento de queratosis actinicas, carcinoma epidermoide y carcinoma basocelular. Los fundamentos basicos de la terapia fotodinamica son descritas. La preparacion y modo de empleo de la terapia fotodinamica con 5-metil aminolevulinato (TFD-MAL) son detallados. Los estudios clinicos que fueron realizados con terapia fotodinamica para el tratamiento de queratosis actinicas, carcinoma epidermoide y carcinoma basocelular son mencionados. Diferentes agentes fotoinducibles y otras opciones terapeuticas vigentes de primera linea son comparados. La TFD-MAL ha tenido la ventaja de presentar menos efectos secundarios y los mismos han sido mas tolerables que el nitrogeno liquido y el 5 fluoracilo. La TFD-MAL ha sido considerada como una opcion eficaz para el tratamiento de la enfermedad de Bowen. El carcinoma epidermoide invasor ha existido sin evidencia para apoyar el uso rutinario de esta terapeutica. Para el carcinoma basocelular superficial, la TFD-MAL ha presentado una alta tasa de curacion y efectos secundarios transitorios y manejables en lesiones extensas y multiples. La TFD-MAL ha sido un tratamiento eficaz y seguro en los pacientes con carcinoma basocelular, para aquellos con profundidad menor de 2mm. La TFD-MAL ha podido jugar un papel importante en el campo de la prevencion con los pacientes inmunosupremidos, particularmente, aquellos que han requerido trasplante y su inmunosupresion ha sido farmacologica. El uso o no de la TFD-MAL, debe ser evaluado individualmente para cada paciente y presentar las caracteristicas idoneas para cada enfermedad que fue citada en esta revision. La terapia fotodinamica con 5-metil aminolevulinato ha sido una modalidad terapeutica de economia considerable, sin embargo, la misma debera ser evaluada en el contexto de numero de consultas y efectos secundarios que han ofrecido las otras modalidades terapeuticasAdditional details
