Accident in nuclear fuel production facility in Tokaimura. Preliminary assessment of reasons, run and consequences

Description
The accident in Japan Nuclear Cycle Department Institute in Tokaimura took place as a result of mistake of personnel responsible for technological run of the nuclear fuel production process. In consequence the critical mass have been exceeded in one of reaction vessels in technological line. The process of rescue has been presented. The radiological consequences being result of the accident for the staff and environment have been assessed
