Separate measurements of compacta and spongiosa density using a transverse rotation scanner

The absorption profile of the distal forearm, which depends on bone mineralisation, was measured using a high resolution microcomputer-controlled transverse rotational scanner with a tightly collimated I-125 photon source and a scintillation counter. Reconstruction of the anatomical structures within the measured area occured in two stages. Subsequently the linear attenuation coefficient for soft tissues, compacta and spongiosa was obtained by a ''least-square'' algorithm. The absorption coefficient of the compacta and spongiosa provided a measure of bone mineralisation. Correlation between the reconstructed images and the calculated results and measurements was tested by calculation. Ten measurements of a model and of in vivo bones resulted in a coefficient of variation of 1.8% and 3.3% for the compacta and 2.5% and 3.8% for the spongiosa. Relative deviation from the standard varied from +1.3% to -1.0%, depending on mineral content. This new, technically simple procedure for the separate determination of the compacts and spongiosa in the distal radius is useful for the early recognition of changes in bone mineralisation which are usually apparent first in the spongiosa. Mit einem hochaufloesenden mikrocomputergesteuerten Transversal-Rotations-Scanner, der eine eng kollimierte 125J-Photonenquelle und einen Szintillationsdetektor bewegt, werden die von der Knochenmineralisation abhaengigen Absorptionsprofile am distalen Unterarm gemessen. In zwei Schritten erfolgt zunaechst die Rekonstruktion der anatomischen Struktur an der Messstelle. Dann werden ueber einen Least-Square-Algorithmus die drei linearen Schwaechungskoeffizienten fuer Weichteile, Kompakta und Spongiosa ermittelt. Die Schwaechungskoeffizienten der Kompakta und Spongiosa dienen als Mass fuer die Mineralisation des Knochens. Die Uebereinstimmung zwischen rekonstruiertem Bild mit den daraus berechneten Ergebnissen und den Messwerten kann durch eine spezielle Fehlerrechnung geprueft werden. Am physikalischen Modell bzw. bei In-vivo-Messungen ergibt sich fuer 10fach-Messungen ein Variationskoeffizient von 1,8 bzw. 3,3% fuer die Kompakta und von 2,5 bzw. 3,8% fuer die Spongiosa. Die relative Abweichung vom Sollwert als Mass fuer die Richtigkeit liegt - in Abhaengigkeit von der Dichte - zwischen +1,3 und -1,0%. Das vorgestellte neue, technisch wenig aufwendige Verfahren zur getrennten Bestimmung der Dichte von Kompakta und Spongiosa am distalen Radius soll zur Frueherfassung von Aenderungen der Knochenmineralisation dienen, die sich im allgemeinen zuerst in der Spongiosa abspielen.
