Description

The author comments the action of the French institute for Nuclear Safety and Protection (IPSN) in analysing the corrective measures implemented by EDF after maintenance incidents which occurred in 1989, in order to improve the safety and quality of maintenance operations, notably during unit outages. She comments the lessons learnt from maintenance incidents which appeared to be due to failure in human behaviour and organisation. She comments the actions implemented in terms of maintenance of redundant equipment, temporary use of special tools and equipment, and requalification of equipment after maintenance
