Localization of a Xenon 133 bolus using a gamma camera during the closing volume measurement

Description
The 'closing volume' concept has mainly been developed since the work of Dollfuss et al. (1967): these authors have shown that, when a normal subject inhales a bolus of 133Xe at residual volume, this bolus locates preferentially in the upper lung; during the subsequent expiration they observed a rise of Xe concentration measured at the mouth, at the very end of the vital capacity, due to the asynchronism of the opening and closure of distal airways. We have aimed at defining more accurately the location of the radioactivity, using a gamma camera, instead of superposed counters. Doing so, one increases the counting points, and the parametric image obtained pictures the concentration distribution of intrapulmonary Xenon more accurately than could be done by counters. La notion de 'volume de fermeture' s'est essentiellement developpee a la suite du travail de Dollfuss et coll. (1967) montrant, au cours de la meme epreuve, la localisation apicale d'un embol de 133Xe inhale au volume residuel, et l'apparition tardive, a la bouche, a la fin de l'expiration suivante, d'un accroissement de radioactivite temoignant de l'asynchronisme d'ouverture et de fermeture des voies aeriennes peripheriques. Nous avons cherche a determiner de facon plus precise la localisation de la radioactivite intra-pulmonaire, en utilisant une gamma camera a la place des compteurs etages employes par les auteurs: on multiplie ainsi les points de comptage externes, et l'image parametrique creee traduit la repartition de concentration du xenon intra-pulmonaire de facon beaucoup plus fine que ne peuvent le faire les compteurs. Chez les huit sujets normaux examines, nous avons montre que la zone d'hyperconcentration du gaz radioactif occupe pratiquement toute la moitie superieure des champs pulmonaires et qu'il existe une limite nette, concave vers le haut entre cette zone et le reste du poumon ou la concentration est pratiquement nulleAdditional details
