Computed tomography for diagnosis of middle-ear-disease - comparison of conventional X-ray procedures and CT

Description
Due to important improvement in detail resolution and the excellent contrast resolution, in the newest computed tomography (CT) units, normal and pathologic structures of the inner and middle ear can be visualized. The clinical value of CT in comparison to standard X-rays and tomography is prospectrively evaluated on the basis of 50 patient examinations. Today CT is unsurpassed in the diagnosis of cholesteatomas and glomus tumors and is superior to all other imaging techniques. Chronic inflammatory diseases of the middle ear can be evaluated in most cases on the basis of standard X-rays and tomography, CT's importance is primarily complementary, except for preoperative cases. Fractures should be visualized firs t on standard X-rays. If questionable complications are involved (e.g., tympanic or intracranial hematomas), CT should be the next step. Conventional tomograms are not necessary. (orig.)
Abstract (German)
Durch die bedeutende Verbesserung im Detailaufloesungsvermoegen bei unveraendert sehr gutem Kontrastaufloesungsvermoegen der Computertomographen (CT) letzter Generation gelingt es normale and pathologische Mittel- und Innenohrstrukturen ausgezeichnet darzustellen. Der klinische Wert dieser Methode im Vergleich zu konventionellen Uebersichtsaufnahmen und der Tomographie wird prospektiv an Hand von 50 Patienten untersucht. Es zeigt sich, dass der CT in der Diagnostik von Cholesteatomen und Glomustumoren allen anderen bildgebenden Methoden ueberlegen ist und daher als bildgebende Erstdiagnostik zu empfehlen ist. Chronische entzuendliche Veraenderungen sind meist mittels Uebersichtsaufnahmen und konventioneller Tomographie in ausreichendem Ausmass darstellbar, der CT kommt - ausser es ist eine Operation gaplant - nur ergaenzende Bedeutung in Einzelfaellen zu. Frakturen sollten zunaechst mit Uebersichtsaufnahmen abgeklaert werden. Falls der Verdacht auf eine Komplikation (z.B. tympanales oder intracranielles Haematom) besteht, sollte eine CT angeschlossen werden. Eine konventionellle Tomographie eruebrigt sich meist.
