Clinical value of thallium 201 in a cardiology service

Description

At present the most widely used element in isotopic cardiology is undoubtedly 201Tl. In the few years since its appearance many publication testify to its growing use in the external detection of coronary thrombosis, the discovery of ischemia exertion, the non-traumatic observation of patients after an aortocoronary bridging operation, the diagnosis of coronary deficiency associated with another heart disease (aorta narrowing, mitral prolapsus, obstructive cardiomyopathy) and in combination with two other radioisotopic methods. The present work is intended as a modest contribution, still very recent, to the critical study of this new technique in all its present aspects. Part one presents the various characteristics responsible for the advantages and limits of 201Tl, then describes the techniques and apparatus used. The production, dosimetry, toxicity and biological behaviour of 201Tl are also discussed. A hundred and twenty-five examinations were performed in the Nuclear Medicine Service of the Limoges UHC between May 1977 and October 1978. The results are analysed in part two. This is followed by a discussion which attempts, in the light of our experience, to situate the place occupied by 201Tl in the range of complementary examinations useful in declared or assumed coronary cases. We then propose an examination procedure and precise indications we believe to be justified, accounting for economic problems before considering the future prospects of myocardium scintigraphy

Abstract (French)

Actuellement, le 201 Tl est sans aucun doute l'element le plus utilise en Cardiologie Isotopique. Depuis son apparition, en quelques annees, de nombreuses publications font la preuve de son utilisation croissante dans la detection externe de l'infarctus myocardique, dans le depistage de l'ischemie d'effort, dans la surveillance non sanglante des patients ayant beneficie d'un pontage aortocoronarien, dans le diagnostic de l'insuffisance coronarienne associee a une autre cardiopathie (retrecissement aortique, prolapsus mitral, cardiomyopathie obstructive), enfin en association avec deux autres methodes radioisotopiques. Notre travail voudrait etre une contribution modeste, sans doute, et encore tres recente, a l'etude critique de cette nouvelle technique dans tous ses aspects actuels. Ceci nous a amene a presenter, dans une premiere partie, les differentes caracteristiques du 201 Tl dont decoulent son interet comme ses limites, avant d'exposer les techniques utilisees ainsi que l'appareillage. La production, la dosimetrie, la toxicite, et le devenir biologique du 201 Tl ont egalement ete decrits. Cent-vingt-cinq examens ont ete realises entre mai 1977 et octobre 1978 dans le service de Medecine Nucleaire du CHU de Limoges. L'analyse de leurs resultats constitue la deuxieme partie. Celle-ci est suivie d'une discussion qui essaie, a la lumiere de notre experience, de situer la place du 201 Tl dans la batterie d'examens complementaires utiles chez le coronarien avere ou suppose. Puis nous proposons un protocole d'examen et des indications precises que nous estimons justifiees, en tenant compte des problemes economiques, avant d'envisager les perspectives d'avenir de la scintigraphie myocardique
