Metabolic radiotherapy of neuroendocrine tumors using DOTATATE labeled with lutetium 177: Lebanese experience about 6 cases with review of the literature

Description
The prevalence of neuroendocrine tumors (NET) is rare. Although some aggressive forms may exist, most NET shows a slow growth. The diagnosis is frequently made at a metastatic stage with more than 50% of hepatic metastasis. The only curative treatment is radical surgery and the other non-surgical therapeutic choices are chemotherapy, hepatic chemoembolization, alpha interferon, cold or non-radioactive somatostatin analogues and, more recently introduced, metabolic or internal radiotherapy with a somatostatin analogue coupled to an appropriate radioisotope. Nowadays, imagining of NET with a 68Ga-DOTATATE PET allows obtaining a better image quality and an effective dosimetry before treating with a somatostatin analogue coupled to an appropriate radioisotope such as 177Lu-DOTATATE. This latter can stabilize a rapidly progressive disease and improve quality of life enough and change it deeply. We are presenting our Lebanese experience by reporting six cases treated with 177Lu- DOTATATE after a PET/CT 68Ga-DOTATATE, and a review of literature. (authors)

La prevalence des tumeurs neuroendocrines (TNE) est rare. Malgre la presence de forme agressive, plusieurs TNE sont de croissance lente et leur diagnostic est tres souvent porte tardivement a un stade metastatique avec plus de 50 % de metastases hepatiques. Le seul traitement curatif est la chirurgie radicale et les differents modes therapeutiques non chirurgicaux utilises chez ces patients sont la chimiotherapie, la chimioembolisation hepatique, l'interferon alpha, les analogues froids ou non radioactifs de la somatostatine et, plus recemment introduite, la radiotherapie metabolique ou interne par un analogue de la somatostatine couple a un radio-isotope adapte. Actuellement, l'utilisation du 68Ga-DOTATATE en TEP dans l'imagerie des TNE permet d'obtenir une meilleure qualite d'image et une bonne dosimetrie pre-therapeutique par un analogue de la somatostatine couple a un radio-isotope adapte tel que 177Lu-DOTATATE. Cette nouvelle modalite therapeutique permet de stabiliser une maladie rapidement evolutive et ameliorer suffisamment la qualite de vie en la modifiant profondement. Nous presentons notre experience libanaise a propos de 6 cas traites par le 177Lu-DOTATATE apres une TEP/TDM 68Ga-DOTATATE, avec une revue de la litterature. (auteurs)Additional details
