Kinetics of HBsub(s) antigen in man

Description

The metabolism of HBsub(s) antigen had been studied in three human volunteers. One had chronic hepatitis and two were silent carriers. The HBsub(s) antigen had been isolated and purified from the plasma of each of the three subjects and, after iodination, reinjected to the same donor. The parameters of plasma kinetics of 131I HBsub(s)Ag have been analyzed according to a two compartmental model on the basis of the radioactivity of TCA precipitate (TP) and immunoprecipitate (IP). The fast initial volume of distribution was approximately equal in the three subjects (46.6ml/kg). The metabolic clearance rate (MCR) of IP was the very same in two subjects but is four times higher in one of the silent carrier. The total renewal time (TRT) was about 3.3 days. Assuming that the HBsub(s) antigen extraction was of the order of 65% the plasma HBsub(s) antigen concentration per liter of plasma would be 12 and 53mg/liter for two silent carriers and 61 mg/liter for the patient with chronic hepatitis. The radioactive efflux from the model (calculated as IP.MCR multiplied by HBsub(s) antigen concentration) was identical for the two silent carriers and 50% higher in the patient with chronic hepatitis. The increase possibly reflects an increased synthesis of HBsub(s) antigen in the patient with chronic hepatitis. The cumulative urinary radioactivity when added to the whole body counting demonstrated that radioactivity was excreted solely in the urine. The ratio of organ counting to precordium counting did not vary significantly with time in all subjects

Abstract (French)

L'etude cinetique de l'antigene HBsub(s) marque a l'iode 131 a ete etudie apres auto-transfusion chez trois sujets volontaires porteurs de l'antigene HBsub(s) de sous-type a2d(w). Deux sujets etaient porteurs apparemment sains, le troisieme un malade atteint d'une hepatite chronique. L'antigene HBsub(s) de chacun des sujets a ete isole du plasma et purifie par ultracentrifugation. Chacun des sujets a recu par voie intraveineuse son propre antigene HBsub(s) marque a l'iode 131 et a subi durant 7 jours les examens suivants: comptage global, comptages externes focalises, prelevements sanguins, prelevements urinaires. Les courbes de la radioactivite plasmatique totale exprimee en pourcentage de la dose et les mesures de la radioactivite urinaire montrent que la radioactivite est eliminee exclusivement par les urines. Les comptages externes focalises ne varient pas significativement avec le temps et ne montrent pas de fixation specifique au niveau des zones controlees, en particulier hepatique. En admettant les rendements d'extraction de l'antigene HBsub(s) a 65%, on en deduit les concentrations plasmatiques d'antigene HBsub(s) qui sont de 12 et 53mg/litre chez deux porteurs apparemment sains et 61mg/litre chez la malade. Le volume de diffusion rapide (V1) est du meme ordre de grandeur chez les trois sujets: 46,6ml/kg. La clearance metabolique (MCR), calculee a partir des fractions immunoprecipitables (IP), est du meme ordre de grandeur chez la malade et chez un porteur sain. Par contre, chez l'autre porteur sain, elle est 4 fois plus elevee. Le Temps de Renouvellement Total (TRT) est de 3,3 jours chez les deux sujets de meme clearance metabolique et de 1,1 jour chez le porteur sain de clearance plus elevee
