FES-PET and breast carcinomas: Current situation in 2021

In theory, FES has characteristics that are quite ideal for molecular imaging of receptors. It is a specific tracer, the uptake of which is well correlated with the expression of ER, labeled with fluorine-18, allowing relatively easy distribution to nuclear medicine departments for a common pathology, with a good tumor-to-background ratio, making it possible to establish a mapping of ER-positive (ER+) lesions and, thus, to highlight an inter-metastatic heterogeneity based on the combination of FDG and FES. The heterogeneity in this case corresponds to the concomitance of metastatic FES/FES+ lesions. On the other hand, the hepatobiliary elimination of FES does not allow the detection of ER+ liver metastases, nor a precise analysis of the abdominal cavity. In addition, the results established from studies with insufficient levels of evidence, this situation being partly linked to its lack of availability, did not allow the development of FES-PET imaging in clinical practice. The lack of sensitivity and specificity to predict a tumor response to hormone therapy, which is now often associated with therapy targeting signaling pathways 'parallel' to ER, represent other limits to its prescription by oncologists. Its sale since March 2017 in France and since early 2021 in the USA and its reimbursement since 2020 in France could at least make it possible to perform FES-PET imaging in patients with initially ER+ metastatic breast cancer or early recurrence after hormone therapy, to map the ER+ lesions when the biopsy is deemed impossible and another line of hormone therapy is considered. Another situation to investigate is that of mammary carcinomas with low metabolic activity, such as infiltrating lobular carcinomas, or in clinical dilemma, in particular in case of equivocal lesions on conventional work-up. Abstract (French)

Le FluoroEStradiol (FES) presente en theorie des caracteristiques assez ideales pour l'imagerie moleculaire de recepteurs. Il s'agit d'un traceur specifique, dont la captation est bien correlee a l'expression des recepteurs aux estrogenes (RE), marque avec du fluor-18, permettant une distribution aux services de medecine nucleaire relativement facile pour une pathologie courante, avec un bon rapport tumeur/bruit de fond, permettant d'etablir une cartographie des lesions RE-positif et, ainsi, de mettre en evidence une heterogeneite tumorale inter-metastatique basee sur la combinaison du FDG et du FES. L'heterogeneite correspond dans ce cas a la concomitance de lesions metastatiques FES-/FES+. En revanche, l'elimination hepatobiliaire du FES ne permet pas la detection de metastases hepatiques RE- positif, ni une analyse precise de la cavite abdominale. De plus, les resultats etablis a partir d'etudes avec des niveaux de preuve insuffisants, cette situation etant en partie liee a son manque de disponibilite, n'ont pas permis l'essor de la TEP-FES en pratique clinique. Le manque de sensibilite et de specificite pour predire une reponse tumorale a l'hormonotherapie, qui est dorenavant souvent associee a une therapie ciblant des voies de signalisation 'paralleles' aux RE, representent d'autres handicaps qui font que son appropriation par les onco-senologues parait actuellement tres hypothetique. Sa commercialisation depuis mars 2017 en France et depuis debut 2021 aux USA et l'obtention d'un remboursement depuis 2020 en France pourrait permettre au moins de proposer aux patients ayant un cancer du sein initialement RE-positif, metastatique ou en rechute precoce apres un traitement par hormonotherapie, d'etablir une cartographie des lesions RE-positif lorsque la biopsie est jugee impossible et qu'il existe une ligne d'hormonotherapie envisageable. Une autre situation a etudier est celle des carcinomes mammaires avec faible activite metabolique, tels que les carcinomes lobulaires infiltrants, ou des cas litigieux, notamment en cas de lesions equivoques lors du bilan d'imagerie.
