Imaging of renal cell carcinoma

Background
Due to the increase in ultrasound investigations and the simultaneous improvement of laboratory tests (detection of blood in urine), nowadays supplementary imaging procedures are used for further clarification of unclear renal lesions. Improved imaging quality of modern ultrasound machines and the introduction of ultrasound contrast media have led to a clear improvement in ultrasound investigations with an increase in the detection rate of renal lesions and also to an improvement in the differentiation of solid and in particular cystic lesions. With the publication of the revised S3 guidelines on renal cell carcinoma of the German Cancer Society, the diagnostic procedures for renal cell carcinoma have been updated.Objective
This article presents the new diagnostic procedures against the background of the currently available literature.
Diagnostic procedures
With the introduction and comprehensive application of multislice computed tomography (CT) systems it now has a much higher temporal resolution, which enables depiction of tumors in the early arterial phase. With these rapid CT scanners the perfusion characteristics of tumors can be documented and addition information can be gathered from the behavior of the contrast medium, which enables further differentiation of tumors. Magnetic resonance imaging (MRI) allows assessment of the perfusion behavior without exposure and enables an extended differentiation of renal space-occupying lesions using T1 and T2 contrast imaging as well as further procedures, such as diffusion-weighted imaging. In extensive tumors with infiltration of the vena cava, MRI allows an improvement in the visualization of the thrombus and can improve planning of surgery for extensive tumors.

Zusammenfassung
Hintergrund
Durch die Zunahme der Ultraschalluntersuchungen und der gleichzeitigen Verbesserung von Labortests (Blutnachweis im Urin) werden heute weiterführende bildgebende Verfahren angewandt, um unklare Läsionen der Nieren weiter abzuklären. Auf der anderen Seite führen bessere Ultraschallgeräte und die Einführung von Ultraschallkontrastmitteln (Ultraschall-KMl) zu einer deutlichen Verbesserung der Ultraschalluntersuchungen mit einer Zunahme an Nachweisraten von renalen Läsionen als auch zu einer Verbesserung der Differenzierung von soliden, aber auch v. a. zystischen Läsionen. Mit der Veröffentlichung der neuen S3-Leitlinien der Deutschen Krebsgesellschaft für das Nierenzellkarzinom wurden die diagnostischen Verfahren beim Nierenzellkarzinom erneuert.
Ziel
Diese Arbeit stellt die neuen diagnostischen Verfahren vor dem Hintergrund der aktuellen Literatur vor.
Diagnostische Verfahren
Die Computertomographie hat durch die Einführung und nun die flächendeckende Anwendung von Mehrzeilensystemen eine deutlich höhere zeitliche Auflösung, was die Darstellung von Tumoren in der frühen arteriellen Phase erlaubt. Hierdurch können Perfusionseigenschaften von Tumoren erfasst werden und so über das KM-Verhalten weitere Informationen gewonnen werden, um die Tumoren weiter zu differenzieren. Die MRT erlaubt ohne Exposition die Beurteilung von Perfusionsverhalten und ermöglicht über T1- und T2-Kontrast sowie weitere Verfahren wie die Diffusion eine weitergehende Differenzierung renaler Raumforderungen. Bei ausgedehnten Tumoren mit Cavazapfen erlaubt die MRT eine Verbesserung der Darstellung des Zapfens und kann so bei ausgedehnten Tumoren die OP-Planung verbessern.
