Complications of brachio-cephalic catheter-angiographies using a non-ionic contrast-medium

In 713 patients 749 catheter-arteriographies of aortic-arch and cerebral vessels were performed using non-ionic contrast media. The patients were examined either with DSA or conventional film techniques. In 80 examinations of 76 patients complications or side-effects were noted, which resolved completely within one hour in 69 cases, showed a duration up to 7 days in 6 cases, and persisted longer than 7 days in 5 patients. Neurological complications were seen in 6,4% of the studies. There was no death. Patients, who received anti-hypertensive drugs for or during angiography and patients with a pathological angiogram had a significantly higher complication-rate. The rising number of patients with atherosclerotic cerebrovascular disease showed an unfavorable effect upon the overall complication-rate. The use of a non-ionic contrast medium did not drastically lower the rate of neurological complications. (orig.)

Es wurden bei 713 Patienten 749 Katheter-Angiographien des Aortenbogens und brachiocephaler Gefaesse mit nicht-ionischem Kontrastmittel in IA-DSA oder Blattfilmtechnik durchgefuehrt. Bei 76 Patienten bzw. 80 Untersuchungen traten unerwuenchte Nebenwirkungen oder Komplikationen auf, die sich in 69 Faellen innerhalb von einer Stunde vollstaendig zurueckbildeten, 6mal bis zu 7 Tagen anhielten und bei 5 Patienten laenger als 7 Tage bestanden. Neurologische Komplikationen ergaben sich bei 6,4% der Untersuchungen, ein Todesfall ereignete sich nicht. Patienten, die im Rahmen der Angiographie Antihypertensiva erhielten sowie Patienten mit angiographisch gesicherten Gefaessveraenderungen wiesen signifikant haeufiger Komplikationen auf. Die Zunahme der Patienten mit atherosklerotischen Gefaessveraenderungen wirkte sich unguenstig auf die Komplikationshaeufigkeit aus. Durch den Einsatz nicht-ionischer Kontrastmittel konnte die neurologische Komplikationsrate nicht entscheidend gesenkt werden. (orig.)Additional details
