Clinical and therapeutic impact of 18F-FDG PET/CT whole-body acquisition including lower limbs in patients with malignant melanoma

Description
Objective. - To assess the added benefit of scanning lower limbs in addition to usual whole-body PET/CT scan in patients with no known or suspected primary or metastatic melanoma involving the lower limbs. Materials and methods. - This is a retrospective study of 133 consecutive patients (189 FDG PET/CT) who underwent FDG PET/CT for staging of melanoma at different time points in the course of disease from October 2005 to July 2009 at Brest University Hospital. Reports of whole-body PET/CT scans including lower limbs were reviewed. PET/CT abnormalities on the lower extremities were tabulated by location and correlated with pathology, other imaging studies and at least a 6-month clinical follow-up. Results. - Among the 189 consecutive PET/CT scans performed in 133 patients, 34 scans in 29 patients highlighted abnormal FDG uptakes considered as equivocal or suggestive of malignancy on lower limbs. In 29 cases, uptakes were located both on lower limbs and on the rest of the body (lung, liver, mediastinal and sub-diaphragmatic lymph nodes, adrenal glands, bone) corresponding to disseminated disease. In five cases, PET/CT uptakes were located only on lower limbs; each pathological uptake corresponded to benign lesions. Lower limbs findings never impacted clinical and therapeutic decision. Conclusion. - Lower limbs additional PET/CT acquisition appears to offer poor additional benefit with no unexpected solitary lesion detected and routine skull base to upper thigh images may be sufficient for this subset of patients. Objectif. - Evaluer le benefice a realiser une acquisition sur les membres inferieurs, complementaire du champ classique en TEP/TDM, chez des patients porteurs de melanome n'interessant pas les membres inferieurs. Patients et methodes. - Il s'agit d'une etude retrospective comprenant 133 patients qui ont beneficie d'une ou plusieurs TEP/TDM au 18F-FDG corps entier (189 TEP/TDM) dans le bilan de melanome, d'octobre 2005 a juillet 2009 au CHRU de Brest. Les interpretations initiales des TEP/ TDM ont toutes ete reprises et les anomalies repertoriees, notamment celles concernant les membres inferieurs. Les anomalies de fixation etaient classees en fonction de leurs localisations et confrontees a une autre imagerie, une analyse histologique si possible et a au moins six mois de suivi clinique. Resultats. - Parmi les 189 TEP/TDM pratiquees sur 133 patients, 34 (correspondant a 29 patients) ont mis en evidence des fixations anormales de FDG evocatrices de malignite sur les membres inferieurs. Dans 29 cas, il y avait des hyperfixations a la fois sur les membres inferieurs et sur le reste du corps (poumons, foie, ganglions mediastinaux et sous-diaphragmatiques, surrenales, os), correspondant a une maladie disseminee. Dans cinq cas, ces anomalies etaient isolees aux membres inferieurs et correspondaient toutes a des lesions benignes. La mise en evidence d'hyperfixations isolees sur les membres inferieurs n'a jamais modifie la decision therapeutique. Conclusion. - Le champ de vue supplementaire centre sur les membres inferieurs ne semble apporter aucun benefice, en raison de l'absence de lesion isolee maligne a ce niveau, et l'acquisition classique de la base du crane a la racine des cuisses parait suffisante pour cette categorie de patients.
