CT assessment of splenic involvement by Hodgkin's disease and Non-Hodgkin's lymphoma

The experience at Stanford University Medical Center (SUMC) with computerized tomography (CT) for determination of splenic involvement by Hodgkin's disease (HD) and non-Hodgkin's lymphoma (NHL) between 1978 and 1982 is presented. Ninety-eight patients had CT during their staging work-up prior to laparotomy and splenectomy. Based on the presence of detectable parenchymal defects before and after intravenous water soluble contrast media, CT sensitivity, specificity and accuracy was 2%, 98% and 54%, with little difference between Hodgkin's disease and non-Hodgkin's lymphoma. Based on weight criterion, the comparable accuracy data was 56%, 72% and 64%. We conclude that CT scanning with and without water soluble contrast media is of no value in detecting splenic involvement by lymphomas, and should not be relied upon when exact knowledge about presence of disease in this organ is needed for further treatment decisions. Abstract (German)
Die Wertigkeit der Computertomographie (CT) zur Erfassung des Status der Milz bei Morbus Hodgkin und non-Hodgkin-Lymphom wurde retrospektiv untersucht. Von 1978 bis 1982 hatten 98 neudiagnostizierte Lymphompatienten am Stanford University Medical Center CT des Abdomens zur Stadieneinteilung vor Durchfuehrung von Laparotomie und Splenektomie. Auf der Basis nachweisbarer Parenchymdefekte vor und nach intravenoeser Kontrastmittelgabe als Kriterium fuer Befall fand sich eine Sensitivitaet, Spezifitaet und zusammenfassende Treffsicherheit der CT von 2%, 98% und 54%. Auf der Basis von Gewicht als Kriterium fuer Befall (200 g) fanden sich entsprechende Ergebnisse von 56%, 72% und 64%. Die CT mit und ohne intravenoeser Kontrastgabe erweist sich fuer die Befundung der Milz bei der primaeren Stadieneinteilung vom Lymphompatienten als ungeeignet. Wenn genaue Kenntnis ueber den Status der Milz erforderlich ist, sollten Therapieentscheidungen nicht vom Ergebnis der CT der Milz abhaengig gemacht werden. (orig.)Additional details
