Comparison of systolic property and diastolic property in patients with myocardial infarction

Description
In the present study we evaluated systolic and diastolic property in 10 healthy controls and 24 patients with myocardial infarction (MI). To assess systolic negative maximum dv/dt/enddiastolic count (EDC) (maxS), ejection fraction (EF) and diastolic positive maximum dv/dt/EDC (maxD), the equilibrium method of cardiac radionuclide angiography was used in the resting state. The mean value (+-SE) of maxS and maxD for each group were as follows; in healthy controls maxS was 3.2 +- 0.08/sec, maxD was 3.4 +- 0.17/sec, in one MI group (EF >= 50%) maxS was 3.0 +- 0.13/sec, maxD was 2.5 +- 0.12/sec, and in another MI group (EF < 50%) maxS was 2.1 +- 0.14/sec, maxD was 1.2 +- 0.16/sec. MaxD was significantly higher for healthy controls than MI group (EF >= 50%) (P < 0.01), maxS was no statistically significant difference between healthy controls and MI group (EF >= 50%) (P < 0.01). Radionuclide angiographycal four subsets (designated R) were defined that correlated with maxS and maxD. In subset R - I (maxS >= 2.5/sec and maxD >= 2.5/sec) all healthy controls and 6 patients were contained. Mean value (+-SE) of EF was 60.0 +- 5.2%. In subset R - II (maxS >= 2.5/sec and maxD < 2.5/sec) 7 patients were contained. Mean value (+-SE) of EF was 56.9 +- 4.7%. In subset R-III (maxS < 2.5/sec and maxD < 2.5/sec) 10 patients were contained. Mean value (+-SE) of EF was 37.0 +- 3.1%. In subset R-IV (maxS < 2.5/sec and maxD >= 2.5/sec) only one patient was contained. EF was no statistically significant difference between R-I and R-II, but maxD was significantly lower for R-II than R-I. In conclusion our results suggested that maxD was more sensitive and predictive than the others, in other words, diastolic property was more sensitive and predictive to evaluate the abnormalities of cardiac function.
