SLAP lesion of the shoulder. MRI evaluation

Description
Purpose of this work is to assess MR potentials in the evaluation of superior glenoid labrum disease and possible associated conditions of the rotator cuff and of the anterior mechanism of the shoulder. 51 patients ( age range 18 to 53 years) with a diagnosis of anteroposterior lesion of the superior glenoid labrum were retrospectively evaluated. MR examinations were performed with a 0.2 T permanent magnet and a dedicated coil, using T1- and T2-weighted SE sequences on mostly coronal-oblique planes. Slice thickness was 4 mm. In 8 cases, the examination was completed with intra-articular injection of contrast agent. Twenty-eight patients were submitted to surgery (arthrotomy in 7 cases; arthroscopy in 21 cases). MRI demonstrated 5 cases of superior labrum irregularities at the level of its glenoid insertional portion (type I lesion); 6 cases of detachment of the superior portion of the labrum (type II); 9 cases of bucket handle tear of the superior labrum with involvement of the insertional portion of the long head of the biceps tendon (type III); 8 cases of superior labrum tear extending within the long head of the biceps tendon (type IV). Subsequent surgery always confirmed the presence of associated lesions, while the superior labrum lesion was not confirmed in 3 patients. In 4 cases, surgical findings provided a different classification of the lesion type than MRI. MRI can be a valuable diagnostic technique in type III and IV lesions of the superior glenoid labrum. It often provides important information about the possible presence of associated diseases, especially of the rotator cuff, which are helpful for treatment planning
Abstract (Italian)
Scopo di questo lavoro e' analizzare le potenzialita' della RM nella valutazione della patologia del cercine glenoideo superiore e delle eventuali lesioni associate della cuffia dei rotatori e della cinghia funzionale anteriore. 51 pazienti (con eta' compresa tra 18 e 53 anni), nei quali era stata posta diagnosi di lesione antero-posteriore del cercine glenoideo superiore, sono stati valutati retrospettivamente. L'esame RM e' stato condotto con apparecchio da 0,2 tesla permanente e bobina dedicata, utilizzando sequenze SE T1 e GE T2 pesate secondo piani prevalentemente coronali-obliqui e con spessori dello strato di 4 mm. In 8 casi l'indagine e' stata completata con l'iniezione intraarticolare di mdc, mentre solo 28 pazienti sono stati sottoposti a intervento chirurgico (artrotomico in 7 casi e artroscopico in 21). La RM ha individuato complessivamente: 5 irregolarita' del cercine superiore a livello della sua porzione inserzionale glenoidea (lesione di tipo I); 6 disinserzioni del cercine nella sua porzione superiore (tipo II); 9 casi di rottura del cercine superiore a manico di secchio con interessamento della porzione inserzionale del capo lungo del bicipite (tipo III); 8 casi di rottura del cercine superiore che si prolunga nello spessore del tendine del capo lungo del bicipite (tipo IV). Il controllo chirurgico successivo ha sempre confermato le lesioni associate, mentre ha escluso la lesione del cercine glenoideo superiore in 3 pazienti. In altri 4 casi il riscontro operatorio ha permesso di classificare in maniera diversa rispetto alla RM il grado della lesione. I risultati dimostrano che la RM puo' offrire un valido contributo nella diagnosi dei tipi III e IV di lesione del cercine glenoideo superiore fornendo spesso decisive informazioni sulla presenza di eventuale malattia associata, specie della cuffia dei rotatori, elementi di sicuro valore nella corretta pianificazione dell'approccio terapeuticoAdditional details
