Description
The determination of 17-ketosteroids (or 17-oxysteroids) in the 24-hourly urines is one of the exploratory hormone analyses most frequently requested of biologists. It is a useful guide in the diagnosis of hyperandrogeny in women, hypogonadism in men, androgenic tumour in both sexes or enzyme blockage leading to congenital hypoplasia of the suprarenals. The analytical bases of this determination are complex: the different steroids measured as a whole possess chromogenic powers varying from 1 to 4; their proportions depend on sex, age or pathological state; the use of Allen's correction to eliminate non-steroid chromogens is not always justified; the technical details of Zimmermann's reaction vary from one laboratory to another, etc. However it is possible to detect frankly pathological states outside the very loose norms usually accepted. The number of determinations the specialized biologist is asked to perform is large enough to justify partial automation at least. Two quite different techniques are described and compared: one sequential, reproducing the standard manual method in the ether phase with Allen's correction; the other a continuous-flux aqueous-phase method without Allen's correction

Le dosage dans les urines de 24 heures des 17-cetosteroides (ou 17-oxosteroides) est l'une des explorations hormonales de depistage les plus frequemment demandees au biologiste. Elle permet d'orienter le diagnostic d'une hyperandrogenie chez la femme, d'un hypogonadisme chez l'homme, d'une tumeur androgenique dans les deux sexes ou encore d'un bloc enzymatique provoquant une hyperplasie congenitale des surrenales. Les bases analytiques de ce dosage sont complexes: les differents steroides doses en bloc ont des pouvoirs chromogenes variant de 1 a 4; leurs proportions varient suivant le sexe, l'age ou l'etat pathologique; l'usage de la correction d'Allen, pour eliminer les chromogenes non steroidiens ne se justifie pas toujours; les modalites techniques de la reaction de Zimmermann varient d'un laboratoire a l'autre, etc... Neanmoins, les normes tres larges habituellement admises permettent de deceler les etats franchement pathologiques. Le grand nombre de dosages demandes au biologiste specialise en justifie l'automatisation au moins partielle. On a presente en les comparant deux techniques tres differentes; l'une sequentielle, reproduisant la methode manuelle classique en phase etheree avec correction d'Allen, l'autre en flux continu sans correction d'Allen, en phase aqueuseFiles
