MR study of acute myocardial infarction with injection of Gd-DOTA (Fifteen patients)

We studied 15 patients 4 to 8 days after myocardial infarction by using ECG gated MR before and after administration of 0.2 mmol/kg Gd-DOTA. The diagnosis in each patient was confirmed by electrocardiographic criteria, elevated levels of fractionated creatine kinase (CK) isoenzyme, thallium scintigraphy, ventriculography and coronarography. T1-weighted, spin-echo images, were obtained before and immediately after injection of Gd-DOTA and were repeated 15 min later. The site of infarction was visualized in 10 patients as an area of high signal intensity after the injection of Gd-DOTA. Contrast between normal and infarcted myocardium was greatest 15 min after injection. Three patients were excluded because of failure to acquire adequate MR studies. In 2 other patients, the infarct were not detected. Before injection of Gd-DOTA, only 2 infarcts were detected. These results suggest that Gd-DOTA can improve MR visualization and detection of acute myocardial infarction
Abstract (French)
Quinze patients sont etudies en IRM echo de spin entre le quatrieme et le huitieme jours apres un infarctus myocardique, avant et apres injection intraveineuse de Gd-DOTA. Dans chaque cas, le diagnostic d'infarctus est confirme par les criteres electrocardiographiques, l'elevation enzymatique, la scintigraphie au thallium, la ventriculographie et par la coronarographie. Les images, ponderees T1, sont obtenues avant et immediatement apres l'injection de Gd-DOTA de meme que 15 minutes plus tard. Sans injection de Gd-DOTA, l'infarctus n'est detecte que dans deux cas comme une zone de signal legerement plus intense que le myocarde normal. Apres injection de 0,2 mmol/kg de Gd-DOTA, l'infarctus est visible et bien delimite chez 10 patients. La difference de contraste entre le myocarde normal et infarci est maximal environ 15 minutes apres injection. Trois patients ont du etre exclus car les images obtenues etaient de trop mauvaise qualite. Dans deux cas, l'infarctus n'est pas visible. Ces resultats suggerent que le Gd-DOTA peut ameliorer la detection et la delimitation de l'infarctus aigu du myocardeAdditional details
