Correlation of myocardial perfusion scintigraphy and coronary angiography

Discrepancy in findings of coronary angiography and myocardial perfusion-SPECT mostly reflect the hemodynamic effect of the pathological coronary morphology rather than misinterpretation by welltrained nuclear medicine physicians. Thus, perfusion scintigraphy gives a valuable completion of the angiographic findings. Consequently, forecast of coronary stenoses loses importance. The intention of cardiovascular medicine focuses more and more on functional statements. The detection of ischemia is not enough for correlated evaluation of SPECT/PET and coronary angiography. At least, assignment of perfusion defects and vascular territories is necessary. Quantitative evaluation and display of bull's eye are helpful and superior to visual interpretation. Due to the variability of the coronary vascular tree, assignment of angiographic and scintigraphic findings may sometimes remain ambiguous. Moreover, in patients with multivessel disease the detection of less stenosed vessels can be hidden if the stress tolerance is limited by the most severe stenosis. Other factors like morphology, length, and location of a stenosis, collaterals and endothelial dysfunction may also modify myocardial perfusion. Thus 'sensitivity' and 'specificity' of assignment of ischemia to a vascular territory ranges between 62% and 86%. With respect to a high-quality standard of the examinations and to the methodical limitations, head to head comparison of scintigraphic and angiographic images enable evaluation of the hemodynamic consequences of stenoses of a distinct coronary artery. The discrepancy of scintigraphic and angiographic findings in particular can lead to more reliable conclusions than the single examination does with direct impact on decision-making regarding medical or invasive therapy, complete or partial revascularization, and the selection of the vessel which has to be revascularized. Correlation of scintigraphy and angiography enables an individual planning of the therapy procedures with respect to distinct coronary arteries. Current developments forces on 3D-image fusion of SPECT/PET and coronary vascular tree with the intention of a more precise assignment of the two examination modalities. Compared to conventional study evaluation, 3D-fusion image improves reliability of the assignment of scintigraphic and angiographic findings. (orig.)

Diskrepante Befunde zwischen Koronarangiographie und der Myokardperfusions-SPECT resultieren oft nicht aus einer Fehleinschaetzung von seiten des routinierten Nuklearmediziners, sondern reflektieren eher die haemodynamische Auswirkung der veraenderten koronaren Morphologie. Somit ist die Perfusionsszintigraphie eine wertvolle Ergaenzung zur Koronarangiographie. Dies spiegelt sich auch in dem seit einiger Zeit ablaufenden Wandel der Intention der kardiovaskulaeren Nuklearmedizin wider, mit Abwendung von der Vorhersage der Koronarmorphologie hin zur funktionellen Aussage. Der Nachweis einer Ischaemie allein reicht fuer die korrelative Beurteilung von SPECT/PET und Angiogramm nicht aus. Vielmehr sollte eine direkte Zuordnung der Perfusionsstoerung zu einem Gefaessgebiet Beruecksichtigung finden. Quantitative Auswertungen und insbesondere die Bull's-eye-Darstellungen sind hilfreich und der einer visuellen Befundung ueberlegen. Bedingt durch die erhebliche Variabilitaet des Verlaufes der Koronararterien, ist die Zuordnung von Angiogramm und nuklearmedizinischem Befund keineswegs immer eindeutig. Auch kann bei einer Mehrgefaesserkrankung eine Stenose die Belastbarkeit des Patienten limitieren und so die Demaskierung weiterer Stenosen verhindern. Darueber hinaus wirken sich zahlreiche Faktoren, wie z.B. die Morphologie, Laenge und Lokalisation einer Stenose, Kollateralen und Stoerungen der Endothelfunktion, modifizierend auf die Perfusion aus. Die 'Sensitivitaet' und 'Spezifitaet' fuer die Zuordnung einer Ischaemie zum Versorgungsgebiet einer stenosierten Koronararterie liegen deshalb durchschnittlich nur bei 62-86%. Unter Beruecksichtigung der notwendigen Qualitaetsanforderungen an die Untersuchung und der durch die Methode gesetzten Grenzen erlaubt die Zusammenschau von SPECT/PET und Angiographie, ueber die haemodynamische Auswirkung einer bekannten Koronarstenose Aussagen zu treffen. Speziell aus der Diskrepanz der Befunde koennen Rueckschluesse gezogen werden, die ueber die Moeglichkeiten der einzelnen Untersuchung hinausgehen. Hieraus koennen Entscheidungshilfen abgeleitet werden, bezueglich eines konservativen oder invasiven Therapieansatzes, einer kompletten oder teilweise Revaskularisation bzw. des zu revaskularisierenden Gefaesses. Eine Beurteilung der nuklearmedizinischen Daten vor dem Hintergrund der Koronarangiographie ermoeglicht eine individuelle und auf die Koronararterien bezogene Therapieplanung. Neuere Entwicklungen versuchen durch fusionierte 3D-Darstellungen von SPECT/PET und Koronarangiogramm, eine bessere Zuordnung beider Untersuchungsmodalitaeten zu erzielen. Die 3D-Bildfusion bietet gegenueber der konventionellen Betrachtung mehr Sicherheit bei der Zuordnung von nuklearmedizinischen und angiographischen Befunden. (orig.)Additional details
