Scar or prolonged ischemia? Comparison between conventional 201thallium-scintigraphy and 99mTc-MIBI-SPECT at rest

Therefore, 50 patients with defects at Thallium redistribution scintigraphy and angiographically proven coronary artery disease underwent conventional Tl-scintigraphy and in addition Tc-MIBI-SPECT at rest and under medication. 62% of the myocardial areas with defects at redistribution scintigraphy showed an ischemia. With Tl-201, however, the full amount of these ischemic lesions could only be identified in one third of the cases. In 2 third a prolonged ischemia was found, partly only a prolonged ischemia, which only was detectable by Tc-MIBI-SPECT. With respect to the assessment of myocardial viability this means that without Tc-MIBI the amount and extent of scared tissue would be overestimated in 41% of the involved areas. (orig./MG)
Abstract (German)
Bei 50 Patienten mit Defekt im Thallium-Redistributions-Szintigramm und angiographisch gesicherter koronarer Herzkrankheit wurde zusaetzlich zur ueblichen Thallium-SPECT mit Belastungs- und Redistributions-Szintigramm eine Ruhe-Tc-MIBI-SPECT unter antianginoeser Medikation durchgefuehrt. Bei der visuell-qualitativen Beurteilung der Szintigramme fand sich in 62% der Defektareale eine Ischaemie, deren volles Ausmass im konventionellen Tl-SPECT nur in einem Drittel nachweisbar war. Bei zwei Dritteln lag eine prolongiert verlaufende Ischaemie vor, in einem Teil davon alleine eine prolongierte Ischaemie, die nur mit zusaetzlicher Tc-MIBI-SPECT erkennbar war. Hinsichtlich der Vitalitaetsbeurteilung bedeutet dies, dass ohne Tc-MIBI-SPECT Ausmass und Ausdehnung der Infarktdefekte in 41% der betroffenen Myokardareale ueberschaetzt worden waere.
