Observations of super early left ventricular remodeling experimental myocardial infarction

Purpose: Ventricular remodeling is defined as the changes in the shape and size of the entire left ventricle after acute myocardial infarction (AMI). Many investigators have shown that left ventricular remodeling is related to clinical outcomes, including mortality, that represent the natural history, of the heart failure syndrome. The aim of this study was to demonstrate that it is possible to observe super early left ventricular remodeling by 99mTc-MIBI myocardial imaging in the dog model of acute experimental myocardial infarction. Methods: Experimental subjects: Twenty-three healthy mongrel dogs (14-25 kg) of either sex were studied under general anesthesia (sodium pentobarbital, 30 mg/kg). The left anterior descending (LAD) coronary artery was dissected and ligated between the first and second diagonal branches. Seven dogs died of ventricular fibrillation after the LAD coronary artery ligation. The 16 remaining dogs were divided into two groups: Group A (GA) received 99mTc-MIBI myocardial imaging (n=8): Group B (GB) received 99mTc-MIBI myocardial imaging combined with echocardiography (n=8). 99mTc-MIBI myocardial perfusion imaging :Static 99mTc-MIBI myocardial imaging was taken with ADAC Vertex Dual-head SPECT. 99mTc-MIBI kit was manufactured in Syncor, China. Each dog served as its own control, and was scanned by 99mTc-MIBI myocardial imaging and chocardiography at 48-72 hours before ligation. The mean time of the first acquisition was 21.87 ± 11.03 (14-48) minutes post-operatively in GA, 57.63±22.83 (30-99) minutes for 99mTc-MIBI imaging in GB, 26.00±15.07 (12-50) minutes for echocardiography in GB. Acquisition techniques for Gated SPECT: ECG synchronized data collection: R wave trigger, 8 Frames/Cardiac cycle. Images were gathered by rotating the detectors 180 degrees at 6 degrees per frame. Each frame took 40 seconds. The dog position was supine. The images were acquired and recorded for 6 hours following the LAD coronary artery ligation. After 6 hours, the animals were euthanized and their hearts were examined for pathology. SPECT reconstruction parameters: Filter; Butterworth; cutoff 0.35 and order 5: filtered backprojection. Images: short axis, horizontal and vertical long axis. Data analysis: Quantitative analysis: Quantitative Gated SPECT (QGS) software was used for calculating the end diastolic volume (EDV), end systolic volume (ESV) and ejection fraction (EF) Data from 16 animals was used. Qualitative analysis was used to assess shape, size of entire left ventricle, and wall motion which data was from 15 dogs. Echocardiography: HP5500 color Doppler ultrasonography diagnosis system with 12 MHz was used for echocardiography. EDV, ESV and EF were calculated by Simpson's method. Statistics analysis: SPSS statistics software was used for all quantitative data analysis. Results: A typical defect with complete absence of wall motion was found at the apex in all dogs (GA and GB) after LAD ligation, beginning with the first acquisition. Post-operatively, EDV and ESV significantly increased compared with pre-operation values. EF significantly decreased post-operatively. The table below shows quantitative data obtained at the earliest time after ligation. EDV and ESV continued to increase during the 6 hours post-operatively, while EF was just the opposite, decreasing with time. Using continuous gated myocardial imaging, reperfusion was found in 10 dogs (62.5%) on the apex area at 1-3 hours after the defect was originally visualized, when a thinning wall could be seen on the apex defect area. By visual analysis we found structural characteristics of left ventricular remodeling, including infarct expansion with thinning (14/15 animals), lengthening (15/15), regional enlargement (9/15) and distortion (11/15). We also found remodeling of non-infarcted myocardium, including thickening (12/15) and lengthening (13/15) as well as global distortion of shape (15/15) and global ventricular dilatation (14/15). Statistical analysis showed that there were no significant differences between 99mTc-MIBI imaging and echocardiography for measuring EDV, ESV and EF and their changing trends. Conclusion: The experimental data demonstrate that super early left ventricular remodeling may be visualized within 14 minutes after LAD coronary artery ligation. We see immediate left ventricular remodeling, which changes the shape and size of the entire ventricle, and we also find that the myocardial function decreases severely with time. 99mTc -MIBI myocardial perfusion imaging and echocardiography both are effective tools for studying LV remodeling. The results may be considered a reference for the treatment of AMI. (authors)
