Evaluation of different primers and DNA preparations for molecular diagnosis of human African Trypanosomosis

This study was aimed at improving the diagnosis of Human African Trypanosomosis (HAT or sleeping sickness). We used the PCR on blood samples to compare two different methods of extraction/purification of DNA (chelex and DNA easy), and several PCR techniques (TBR 1-2, TgsGP, ESAG 6-7). The comparisons related to subjects taken at the time of medical surveillance in the mid-west of Ivory Coast and Benin: patients (Trypanosomes seen by parasitological tests), subjects with positive serologic tests but negative with parasitological tests (called sero-positives), negative subjects in serology and parasitology, and healthy volunteers living in Europe. The best combination was TBR used with Chelex. Under these conditions, the PCR test is simple, easily usable in a tropical laboratory, more sensitive than the parasitological examinations, and could help with the early detection of the cases, the decision to treat a patient, and with the post-therapeutic follow-up to detect cases of relapse. However, the limits of specificity and its high cost still restrict its use at the research laboratories
Abstract (French)
Dans cette etude, afin dessayer d'ameliorer le diagnostic de la Trypanosomose Humaine Africaine (THA, ou maladie du sommeil), nous avons utilise la PCR sur sang en comparant 2 methodes differentes d'extraction/purification d'ADN (chelex et DNA easy), et plusieurs amorces PCR (TBR 1-2, TgsGP, ESAG 6-7). Les comparaisons ont porte sur des sujets preleves lors de prospections medicales dans le centre-ouest de la Cote d'Ivoire et au Benin: des malades (Trypanosomes vus en parasitologie), des sujets positifs aux tests serologiques mais negatifs aux tests parasitologiques (appeles seropositifs), des sujets negatifs en serologie et parasitologie, et des volontaires sains vivant en Europe. La meilleure combinaison a ete l'amorce TBR utilisee avec le Chelex. Dans ces conditions, la PCR est un test simple, facilement utilisable dans un laboratoire tropical equipe, plus sensible que les examens parasitologiques utilises, et pourrait aider a la detection precoce des cas, a la decision de traiter un malade, et au suivi posttherapeutique pour detecter precocement des cas de rechute. Toutefois, les limites de specificite et son cout encore eleve restreignent encore son utilisation aux laboratoires de recherche. (author)Additional details
