Chapter 13. Adrenal glands
Description
The condition of isotopic methods to the functional and morphological exploration of the adrenal glands is shown, with emphasis on the fact that althought the cortico-adrenal responds to these methods the same does not apply to the medullo-adrenal, which expresses its morphological changes by producing deformations on the cortical image. Funtional tests, mainly directed at the cortico-adrenal, are described first: study of exchangeable sodium and potassium; determination of the plasma concentration and metabolic clearance of some steroid hormones (cortisol, corticosterone, aldosterone); evaluation of the renin activity. These tests are based on competitive analysis and radioimmunological methods. Morphological tests are examined next. Adrenal scintigraphy uses a simple technique (intraveinous administration of 131I 19-iodocholesterol with no special preliminary preparation) which gives good images and is only limited now by the need to avoid over exposure of the gonads to ionising radiations

On a montre l'apport des methodes isotopiques a l'exploration fonctionnelle et morphologique des surrenales, en precisant que si la cortico-surrenale a pu beneficier de ces methodes, il n'en est pas de meme de la medullo-surrenale qui exprime ses modifications morphologiques par les deformations qu'elle imprime sur l'image de la corticale. On a vu tout d'abord les epreuves fonctionnelles qui s'adressent essentiellement a la cortico-surrenale: etude du sodium et du potassium echangeables; determination de la concentration plasmatique, de la production et de la clearance metabolique de quelques hormones steroides (cortisol, corticosterone, aldosterone); evaluation de l'activite renine. Ces epreuves sont basees sur les methodes d'analyse par competition et les methodes radio-immunologiques. On a etudie ensuite les epreuves morphologiques. La scintigraphie des surrenales met en jeu une technique simple (administration par voie IV de 131I 19-iodocholesterol sans preparation prealable speciale) qui fournit de bonnes images et dont l'utilisation ne se trouve actuellement limitee que par le souci de ne pas delivrer aux gonades de doses de radiation ionisantes trop eleveesAdditional details
